Please remember that all patients are responsible for co-pays, deductibles, co-insurance and charges for services rendered at the time of the appointment. The practices of Providence Medical Group accept cash, check and credit cards. Providence Medical Group has negotiated contracts with the following accepted insurance plans. It is still recommended that you call your insurer in advance of your appointment to see if your procedure and provider are covered by your plan. Appointment Notes

  • Patients without insurance cards are considered Self Pay and will be responsible for paying for their services. 
  • If you are more than 15 minutes late to your appointment, you may be asked to reschedule.
  • If it is your first visit, please provide previous medical records and be prepared to fill-out New Patient Information forms in advance of your approintment. You can download these forms from the individual practice pages on this website.
